American Traveler is seeking an experienced RN Case Manager for a 13-week travel contract at an acute care hospital in Arlington, VA, requiring acute care nursing experience and knowledge of health care reimbursement systems.

Details

• Acute inpatient hospital setting focused on care coordination and discharge planning
• Day shift: 8:30 AM – 5:00 PM, 8-hour shifts, 40 hours per week
• Every other weekend required
• Epic EMR is used for documentation

Requirements

• Active VA or Compact RN license required; no pending licenses accepted
• Active certifications must be in hand at time of consideration
• Associate's degree in Nursing (ADN) or higher required
• Minimum 1 year of experience in an acute medical/surgical, ED, or critical care nursing setting
• Experience using an electronic medical record (EMR) system
• Knowledge of health care reimbursement systems including Medicare Advantage, commercial payers, Medicaid, and CMS regulatory rules
• Last 4 digits of SSN and date of birth required for consideration

Additional Information

• Responsible for completing an initial face-to-face assessment within 24 hours of admission to identify each patient's discharge needs
• Collaborates with physicians, nursing staff, utilization review, and the broader care team to develop and communicate transition plans
• Coordinates referrals to in-network post-acute vendors including Home Health, DME, IV Infusion, SNF sub-acute care, and acute rehabilitation
• Works with community providers and ambulatory case managers to facilitate safe, appropriate transitions of care and prevent readmissions
