American Traveler is seeking an experienced RN Case Manager with 5+ years of inpatient acute care case management experience and a current CA RN license for a 13-week travel contract in the San Francisco Bay Area.

Details

• Inpatient acute care hospital setting, Case Management unit
• Day shift schedule: 5x8-hour shifts (8:00 AM–4:30 PM)
• Every other weekend required
• Works collaboratively with physicians, utilization managers, medical social workers, and bedside RNs to facilitate care coordination and transition planning
• Caseload includes complex clinical and social patient situations across the acute care continuum, including potential Emergency Department assignments
• Consistent documentation in the EHR and other electronic software required

Requirements

• Active California RN license required
• Current BLS certification required
• Minimum 5 years of RN Case Management experience required
• Inpatient acute hospital Case Management experience is required — this is a firm manager requirement
• Strong knowledge of utilization management, level of care criteria, CMS/Joint Commission Transitions of Care requirements, and Conditions of Participation (COPs)
• 2 supervisor references obtained within the past year required for consideration
• Copies of all certifications required for consideration

Additional Information

• Responsibilities include patient assessment, care coordination, utilization management, and transition planning throughout the acute care episode
• Collaborates with interdisciplinary teams, payers, community resources, and financial counselors to ensure timely and appropriate patient transitions
• Monitors length of stay, identifies readmission risk, and screens 30-day readmissions to develop effective transition plans
• Addresses social determinants of health and makes referrals to appropriate community and health care resources
• Candidates must reside more than 50 miles from the facility to qualify for travel rates — local candidates may be considered at a different rate
• Former staff or travelers must have been away from the facility for at least 1 year before returning as a traveler
• Only one holiday RTO request will be approved for the duration of the contract
