American Traveler is hiring a BSN-prepared RN for a Case Management position in an acute care hospital in the Bronx, NY, requiring at least 1 year of inpatient case management experience and NY RN licensure.

• Acute care hospital setting, inpatient Case Management department
• Primarily placed on Med-Surg or Telemetry floors following Internal Medicine and Family Medicine teams
• Focuses on discharge planning (not Utilization Review); UR is handled by a separate department
• Typical caseload of 17–25 patients when paired with one Social Worker; pilot floor caseload of 12–17 patients when paired with two Social Workers; must be able to manage at least 20 patients
• Discharge destinations include home, community, SNF, shelter placements, hemodialysis arrangements, and wound VAC (KCI) referrals
• Trauma Level II facility
• EMR: Allscripts (training provided during orientation)
• Monday–Friday, 8:00 AM–4:30 PM, 7.5-hour shifts with a 1-hour lunch break
• Weekend and holiday coverage is rare and only with manager approval; may be asked to cover multiple units if covering a weekend or holiday
• 13-week contract

Requirements

• Active NY RN license required
• BSN required
• Minimum 1 year of inpatient case management experience required
• Current certifications required: BLS, PRI (Patient Review Instrument), and SCREEN Training (both PRI and SCREEN can be completed online through the New York State School of Public Health Albany; each takes a few hours and costs approximately $300)
• Ability to function independently and collaborate with an interdisciplinary team including physicians, social workers, and other care team members
• Strong critical thinking, time management, and communication skills required
• Prior critical care experience preferred
• Familiarity with CarePort, MCG, and/or InterQual criteria preferred; some Utilization Review experience preferred

Additional Information

• Responsibilities center on discharge planning, including rounding with physicians, making referrals, obtaining housing resources and shelter packets, coordinating hemodialysis and wound VAC referrals, and facilitating transitions to home, community, or SNF
• Works collaboratively with a Social Worker on the unit; physicians are responsible for medication reconciliation and pharmacy orders
• Orientation consists of 2 days of hospital orientation (including EMR training) followed by 1–2 weeks with a preceptor depending on competency check-off progress
• Dress code: business casual or any color scrubs except royal blue or maroon (neutral colors preferred); lab coat provided by the facility
• A resource binder is provided to support discharge planning processes
