American Traveler is hiring a clinic LPN with an active MN LPN license and a minimum of 2 years of experience for a 13-week day-shift assignment in Minnesota.

• 36-hour weekly schedule

Requirements

• Active MN LPN license required at start of assignment
• Minimum 2 years of LPN experience required
• DOB and SSN may be required for consideration

Additional Information

• Performs and documents patient assessments appropriate to LPN licensure, including health history, admission processing, and identification of discharge planning and education needs
• Collaborates with RNs and other licensed practitioners to contribute to and modify individualized, patient-centered care plans
• Communicates relevant patient information, including changes in condition and pain reports, to multidisciplinary team members and patients/families in a timely manner
• Implements nursing interventions and prescribed medical regimens safely and documents nursing care accurately
• Participates in health education activities as approved by a licensed practitioner
• Assists with discharge planning, patient and family education, and counseling based on identified needs
