American Traveler is seeking an experienced Home Health/Hospice RN for a 13-week travel contract in rural Colorado, requiring at least 3 years of experience and on-call availability.

• Home health and hospice/palliative care setting in a rural Colorado community
• Caseload includes home health, hospice, and palliative care patients
• Day shift with call requirements of 1-2 nights per week and 1-2 weekends per month
• 13-week contract

Requirements

• Active CO RN license or compact RN license currently in hand
• Current BLS/BCLS certification required
• Minimum 3 years of RN experience required
• Home health and/or hospice nursing experience required
• SSN and DOB required for consideration
• Must have access to a reliable personal vehicle, as the area has no public transportation

Additional Information

• Responsibilities include conducting initial and ongoing patient evaluations, developing and revising plans of care, and providing skilled nursing interventions for home health, hospice, and palliative care patients
• Additional duties include preparing clinical and progress notes, coordinating services, communicating changes in patient condition to the physician and care team, and counseling patients and families
• Colorado Adult Protective Services (CAPS) background check is required
• References are not required but preferred
• This position is located in a rural community without public transportation — personal transportation is essential
