American Traveler is seeking an experienced RN Case Manager for a 13-week travel assignment covering acute inpatient, mother/baby, and women's case management needs in an Indianapolis hospital setting.

• Case Management unit within an acute inpatient hospital setting
• Patient population includes acute inpatient, mother/baby, and women's health case management
• Day shift: 08:00–16:30
• 13-week assignment

Requirements

• Active RN license in Indiana or a compact state currently in hand
• BLS certification required (AHA accepted)
• Minimum 2 years of RN case management experience required
• Must possess a US Social Security Number and provide a valid US home address

Additional Information

• Coordinate the interdisciplinary plan of care for patients from admission through discharge, integrating patient and family preferences
• Monitor patient medical necessity and level of care through ongoing assessments, evaluations, and medical record review
• Advocate for patient resources and removal of barriers to care
• Maintain ongoing communication with the supervisor and care transition team to ensure effective plan implementation and reevaluation
• Serve as a resource for medical record documentation, level of care recommendations, and post-discharge treatment options
• Local candidates are accepted, but candidates residing within the facility's radius may be subject to a reduced pay rate
• Two professional references, a copy of your Driver's License, skills checklist, and copies of all certifications and license verifications are required for consideration
