Description

American Traveler is hiring an LPN/LVN for a long-term care and rehab facility in Rochester, NY, with at least 1 year of LTC experience and Epic EMR familiarity preferred.

Details

• Skilled nursing facility (SNF) setting with a mix of long-term care and rehab patients
• Average daily census of 120 patients
• Patient ratios are 1:20 on days and weekends, and 1:40 on nights
• Required to float among all 3 units within the building
• Primary shift is evenings (3:00 PM–11:30 PM), with possible flexibility to cover nights (11:00 PM–7:30 AM)
• Scheduled for 5x8-hour shifts per week
• Every other weekend (EOW) coverage required, totaling 8 weekend shifts over the 8-week contract
• Epic EMR experience preferred
• Blood glucose monitoring/testing is a required skill

Requirements

• Current LPN/LVN license required if applicable
• Minimum 1 year of LPN experience in a long-term care (LTC) setting
• Current BLS certification required
• Skilled nursing facility or nursing home experience required
• Experience with blood glucose monitoring/testing required
• Candidates must reside more than 75 miles from the facility to be eligible
• 2 professional supervisory references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration
