American Traveler is seeking an experienced RN Case Manager for an adult inpatient Case Management position requiring active Epic experience and current inpatient case management background.

Details

• Day shift schedule: 8:30 AM – 5:00 PM, five 8-hour shifts per week
• No weekend, holiday, or on-call requirements
• Inpatient case management unit within a large acute care hospital setting
• Adult Medicine/Surgery patient population
• Involves coordination of care across the continuum, including transitions to post-acute settings such as nursing facilities, acute inpatient rehab, and local shelters
• Epic EMR experience required
• Experience with post-acute facility referral platforms such as NaviHealth or Allscripts required

Requirements

• Active RN license (Maryland or Compact) currently in hand
• Current BLS (CPR) certification required
• Minimum 2 years of RN experience required
• Current inpatient Case Management experience required
• Experience at a comparable large facility (500+ bed count) required
• Proficiency in Epic EMR required
• Experience with post-acute referral platforms such as NaviHealth or Allscripts required
• SSN and DOB required for consideration
• One professional reference required for consideration

Additional Information

• Coordinates care for a select caseload of patients from triage through safe discharge or transfer to home or post-acute settings
• Proactively communicates with payors to ensure approval of treatment plans and required resources
• Collaborates with the multidisciplinary team and patient/family to facilitate effective transition planning
• Maintains and enters patient information in all departmental systems per operational guidelines
• Local candidates are accepted at the same rate as travelers
• No medication administration responsibilities; medication competency testing is not required
