American Traveler is seeking an experienced RN for a night shift Emergency Department position at a Critical Access Hospital, requiring at least 2 years of ER experience and multiple certifications including ACLS, BLS, TNCC or ATCN, and PALS or ENPC.

• Critical Access Hospital (CAH) Emergency Department with 5 beds, including a 2-bed trauma bay and a dedicated triage area
• Primarily adult and geriatric patients with some pediatric cases; common diagnoses include CHF, COPD, pneumonia, and abdominal pain
• Average patient volume of approximately 20 patients per day
• Nurse-to-patient ratios vary by acuity: 1 RN on days, 1 RN on nights, and 1 mid-shift RN (11:00–23:30)
• Common drips include cardiac, insulin, nitroglycerin, heparin, and antibiotics; CRNAs manage sedation and intubation
• RNs are responsible for all breathing treatments, ABGs, and EKGs; RT is not in-house
• Respiratory equipment includes CPAP, high-flow oxygen, and ventilators (managed until transfer); CRNAs handle intubation
• RNs must be able to independently read and interpret cardiac monitors
• RNs typically draw labs via IV start; phlebotomy available as backup
• X-ray and CT available in-house 24/7; RNs transport all patients except critical cases
• Charting via Cerner EMR
• 12-hour night shifts, 18:00–06:30, on a block schedule rotation (three nights on, five nights off, three nights on, three nights off)
• Every other weekend required
• Holiday coverage required when holidays fall on the scheduled rotation
• May float to acute care to assist with sick patients or general support when the ER census is low

Requirements

• Active WA state or compact RN license required
• Minimum 2 years of Emergency Department (ED) experience required
• Required certifications: ACLS, BLS, TNCC or ATCN, and PALS or ENPC
• NRP certification preferred
• Must be able to work independently in a high-stress environment
• Ability to read and interpret cardiac monitors independently
• Cerner EMR experience beneficial

• Travelers will perform triage independently
• Hold patients are intermixed in the ER and cared for by ER nurses
• No dedicated psych or seclusion rooms available
• No in-house security 24/7
• No nurse aides for general care; EMTs may assist during codes
• Charge nurse is shared with inpatient and will assist in the ER during high-volume periods
• ED physician coverage includes either a mid-level provider or an MD/DO; an MD/DO is available on-call as backup when a mid-level is staffing
• Orientation is 1 day with flexibility to extend if needed
• Any scrub color is acceptable
• 13-week assignment
