American Traveler is hiring an experienced Home Health RN with OASIS documentation experience for a 13-week assignment requiring a Vermont or compact RN license.

Details

• Home health setting serving a patient population with a variety of skilled nursing needs
• Caseload includes patients requiring high-tech skills such as IV therapy and maintenance, wound VAC, and complex wound care
• Functions as Case Manager for assigned patients, managing caseloads and visit times
• OASIS documentation is a core component of this role
• Uses McKesson for clinical documentation
• Schedule is Sunday through Thursday, 8am to 4pm
• Some on-call responsibilities required

Requirements

• Active Vermont (VT) or compact RN license required
• Minimum 2 years of RN experience required
• OASIS experience required
• Proficiency with McKesson EMR
• High-tech nursing skills required, including IV use and maintenance, wound VAC, and complex wound care
• DOB and SSN required for consideration

Additional Information

• Develops and manages individualized plans of care in collaboration with patients, caregivers, and interdisciplinary team (IDT) members
• Delegates tasks to and collaborates with LPNs within their scope of practice, reviewing documentation and visit notes accordingly
• Participates in IDT meetings within five days of patient admission and every 15 days thereafter
• Conducts patient and caregiver education on topics including medication side effects, treatment options, and discharge instructions
• Coordinates care with physicians, providers, and vendors to support the recommended plan of care
• Identifies and communicates interdisciplinary needs such as physical therapy and social work in a timely manner
• Participates in monthly peer review audits
• Coordinates safe patient discharges and transfers, ensuring appropriate paperwork is completed
