American Traveler is hiring an experienced RN for a Home Health Case Manager position in the DuPage County area of Illinois, requiring active IL licensure and prior home health experience.

Details

• Home health setting serving patients in Naperville, Lisle, Bolingbrook, Woodridge, Darien, and the greater DuPage County area
• Patient population consists primarily of adults, with occasional adolescent patients; common diagnoses include stroke and cognitive impairments
• Caseload of approximately 5–6 patient home visits per day
• Skilled procedures include wound care, pleural drain management, central line care, IV/venipuncture, IV infusion via elastomeric pumps, and enteral feeding via Kangaroo pumps
• EMR: Epic (experience preferred)
• Equipment includes Sapphire IV pumps; a company cell phone, laptop, and trunk supplies are provided
• Day shift, Monday–Friday, 8:00 AM–5:00 PM (40 hrs/week)
• Weekend rotation required approximately every 3rd–4th weekend
• No on-call or holiday requirements
• 13-week contract

Requirements

• Active IL RN license (IL or Compact) required
• Current BLS certification required
• Valid driver's license and personal vehicle with auto insurance required
• Minimum 1 year of home health nursing experience required; most recent experience must be within the last year
• Strong skills in time management, effective communication, patient/family education, comprehensive assessment, care planning, and organizational skills
• At least 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN
• Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration

• Responsibilities include comprehensive patient assessment, individualized care planning, patient and family education, and skilled clinical interventions in the home setting
• Supported by a team of RNs, an education team, and facility leadership
• Orientation includes a new employee orientation (NEO) plus 1–2 weeks of department-specific orientation
• Nice-to-have skills include 5+ years of home health experience, Epic proficiency, wound care experience, IV infusion experience, peripheral blood draws, and central line management
• Professional attire required (no scrubs)
• RTO requests must be approved by the manager
• First-time travelers will not be considered for this position
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months to be eligible
