American Traveler is hiring an experienced RN for a Home Health Case Manager position in the western Chicago suburbs, requiring prior home health experience and an IL RN license.

Details

• Home health setting covering a defined Patient Service Area (PSA) in the western Chicago suburbs, including communities such as Westchester, Maywood, Bellwood, Cicero, Berwyn, LaGrange, Brookfield, and surrounding areas
• Office is based in Oak Brook, IL
• Adult patient population with common diagnoses including stroke and cognitive impairment; occasional adolescent patients
• Average caseload of 5–6 patients per day
• Day shift, Monday–Friday, 8-hour shifts (0800–1700)
• Every 3rd weekend rotation required
• No on-call requirements
• EMR: Epic (experience preferred)
• Equipment includes Sapphire IV pumps and Kangaroo pumps; trunk supplies, cell phone, and laptop provided when on-site

Requirements

• Active IL RN license or Compact RN license currently in hand
• Current BLS certification required
• Minimum 1 year of home health nursing experience required, with most recent experience within the last year
• Valid driver's license, personal vehicle, and current auto insurance required
• Strong skills in time management, patient/family education, comprehensive assessment, and care planning
• Proficiency in wound care, pleural drain management, central line care, IV/venipuncture, and peripheral blood draws
• 1 professional reference from a Supervisor, Manager, Director, or Charge RN collected within the last year required for consideration
• Candidate's middle name or initial, date of birth, and last 4 digits of SSN required for consideration

• Responsibilities include comprehensive patient assessments, care planning, patient and family education, and skilled clinical interventions in the home setting
• Supported by a team of RNs, an Education Team, and Leadership
• Orientation includes New Employee Orientation (NEO) followed by 1–2 weeks of department-specific orientation
• Professional attire required (no scrubs)

• No holiday coverage expectations
• RTO requests must be approved by the manager
• Local candidates are accepted
