American Traveler is seeking an experienced RN for a Hospice/Palliative Care Case Manager position in Loudoun County, VA, requiring a minimum of 1 year of recent hospice experience and a valid VA or compact RN license.

Details

• Home hospice setting serving Loudoun County, VA, with a territory of up to a 60-mile radius from the hub/office
• Geriatric end-of-life care patient population
• Caseload of approximately 5-7 patients per day (to be confirmed during interview)
• Case management role requiring flexibility across the assigned territory
• Day shift schedule, Monday through Friday, 8:00 AM – 5:00 PM (5x8-hour shifts)
• No on-call responsibilities
• EMR: Netsmart

Requirements

• Active RN license (VA or Compact) currently in hand
• Current BLS certification required
• Minimum 1 year of recent hospice nursing experience required
• Case management experience required
• Completion of both required skills assessments
• Two professional supervisory references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration

Additional Information

• Provide case management and end-of-life nursing care to geriatric hospice patients across an assigned geographic territory
• Timely documentation and communication are essential responsibilities of this role
• iPad provided for use in the field; personal cell phone is not required
• Mileage reimbursed at the IRS standard rate
• Scrubs required in any color; jeans are not permitted
• Local candidates are accepted and eligible for the travel pay rate
