American Traveler is seeking an experienced RN for a Home Health position supporting a PACE (Program of All-inclusive Care for the Elderly) program in Grand Junction, CO, with a Monday–Friday daytime schedule.

Details

• Home health setting serving elderly participants enrolled in a PACE (Program of All-inclusive Care for the Elderly) program
• PACE is a federally and state-funded program providing comprehensive, coordinated, whole-person care to help seniors live safely at home
• Patient population consists of adults aged 55+ with complex, ongoing healthcare needs
• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• 13-week contract

Requirements

• Active CO RN license or a compact RN license currently in hand
• Minimum 2 years of recent nursing experience in a hospital or other acute care setting
• Current CPR/BLS certification required
• Valid driver's license, reliable personal transportation, and proof of required automobile insurance
• Proficiency with electronic health record (EHR) systems and Microsoft Office applications
• Strong interpersonal, verbal, and written communication skills
• Ability to prioritize, manage competing demands, and follow through consistently

Additional Information

• Promotes and maintains participant health through education, counseling, and coordination of preventive and rehabilitative services in the community
• Assesses health needs, develops and implements nursing care plans, and provides direct nursing care
• Collaborates with an interdisciplinary team to deliver coordinated care and conducts ongoing re-evaluation of participant and family needs
• Coordinates scheduling, communication, and documentation to ensure efficient home care service delivery
• Home health and/or long-term care (LTC) experience is preferred
• DOB and SSN may be required for consideration
