Description

American Traveler is seeking an experienced RN or LMSW for an Integrated Case Manager position requiring 5+ years of case management experience, including recent acute inpatient case management, and an active Michigan RN or LMSW license.

Details

• 13-week contract working 40 hours per week, 8 hours per day
• Services provided across ambulatory, inpatient, and health plan settings
• Caseload includes patients who have experienced critical events or complex diagnoses requiring extensive resource utilization and care coordination
• No blocked scheduling permitted
• Travelers may be required to float to a nearby sister hospital

Requirements

• Active, unrestricted Michigan RN license or Michigan LMSW license required
• BSN or MSW required
• Minimum 5 years of case management experience required
• Minimum 2 years of recent acute inpatient case management experience required — telephonic, utilization review, and psychological case management experience do not satisfy this requirement
• Discharge planning experience required
• Strong verbal communication and written documentation skills
• Proficiency with computer systems and electronic documentation
• Knowledge of Medicare and Medicaid regulations, clinical practice guidelines, and case management principles
• Understanding of social determinants of health and their impact on patient wellbeing
• Experience in discharge planning, home health care, rehabilitative medicine, community health, or managed care preferred

Additional Information

• Conducts comprehensive biopsychosocial assessments and develops personalized, patient-centered care plans
• Engages patients and families through advocacy, health education, resource identification, and care facilitation
• Applies motivational interviewing and self-management techniques to help patients overcome barriers to care goals
• Provides counseling related to treatment decisions, end-of-life issues, and Advanced Care Planning
• Coordinates safe transitions between care settings and advocates within health plan benefit structures
• Facilitates interdisciplinary care conferences and collaborates with complex medical and social care teams
• Facilitates interventions in cases involving child abuse and neglect, domestic violence, elder abuse, and sexual assault
• Limited on-site training is provided — candidates must be able to adapt quickly and work independently
• Online orientation modules must be completed and passed prior to start, typically conducted on-site one week before the start date
• COVID vaccination is mandatory for this position
• Any time-off requests made after an offer is extended must be discussed with management once the assignment begins
