American Traveler is hiring an experienced RN for a Home Health/Hospice position in rural Colorado requiring a minimum of 3 years of experience and call availability.

• Home health and hospice/palliative care setting in a rural Colorado community
• Caseload includes home health, hospice, and palliative care patients
• Day shift with call requirements of 1-2 nights per week and 1-2 weekends per month
• 13-week contract

Requirements

• Active CO RN license or compact RN license currently in hand
• Current BLS/BCLS certification required
• Minimum 3 years of RN experience required
• Home health and/or hospice nursing experience required
• Must have access to a reliable personal vehicle, as the area has no public transportation
• SSN and DOB required for consideration

Additional Information

• Responsibilities include conducting initial evaluation visits, developing and revising individualized Plans of Care, administering skilled nursing services, preparing clinical and progress notes, and coordinating care across the care team
• Involves counseling patients and families, communicating patient condition changes to physicians and other personnel, and supervising and teaching other nursing personnel
• Colorado Adult Protective Services (CAPS) background check is required
• References are preferred but not required
