American Traveler is seeking an experienced RN for a Home Hospice Supportive Care Team (SCT) position in Illinois, requiring a minimum of 2 years of home health or home hospice experience and an active IL RN license.

Details

• Home hospice setting — all patient care takes place in patients' homes across an assigned South 1 Territory (specific counties and zip codes apply)
• Supportive Care Team (SCT) unit focused on after-hours triage and PRN visits, including symptom management, fall assessments, and death visits; some issues may be resolved by phone without an in-person visit
• Patient population includes individuals with failure to thrive, advanced cancers, end-stage COPD, end-stage disease processes, and terminal illness
• RN-to-patient ratio of 0–6 patients per shift, depending on nightly call volume
• 12-hour night shifts, 2000–0800, three shifts per week (36 hours/week)
• Every other weekend rotation required
• Holiday coverage required: one summer holiday and one winter holiday
• No on-call responsibilities outside of scheduled shifts
• May occasionally float slightly outside of the assigned territory when geographically closer to a call; floating never exceeds one hour from the home territory
• EMR: Epic (Rover); Epic experience preferred
• Equipment includes Sapphire IV pump (rarely encountered in home setting)

Requirements

• Active Illinois (IL) RN license required
• Current BLS certification required
• Valid driver's license and personal vehicle with active auto insurance required
• Minimum 2 years of home health or home hospice experience required
• First-time travelers with case management experience in the home setting may be considered
• Proficiency in focused assessments, critical thinking, wound care, foley catheter exchange, feeding tube troubleshooting, post-fall assessments, and narcotic medication administration
• Comfort with death visits, communication with medical examiners or coroners, and identifying potential drug diversion in the home
• Minimum 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the past year required for consideration
• Candidate DOB and last 4 digits of SSN required for consideration
• Candidate middle name or initial required for consideration

Additional Information

• Perform after-hours home visits for symptom management, fall assessments, death visits, and other PRN needs within the assigned territory
• Lead RN available 24/7 for support; travelers are expected to update the Lead RN after each visit and provide a report
• Additional SCT team members are accessible by phone throughout the shift
• Badge, laptop, cell phone, and trunk kit are provided upon onsite orientation
• Orientation includes new employee orientation (NEO) at the territory office followed by 3 shifts (approximately 1 week) with an SCT nurse
• Schedule is generally set; manager will discuss specifics during the interview process
• RTO requests must be submitted through and approved by the manager
• Navy blue scrubs required
