American Traveler is seeking an experienced RN for a Care Coordinator (Case Management) position in an acute care setting, requiring recent California Dignity Health hospital experience in the same role and a valid CA RN license.

Details

• Acute care hospital setting in Sacramento, California
• Care Coordinator unit focused on case management
• Diverse patient population including cardiac, surgical, oncology, and critical care
• Caseload of approximately 15–25 patients per day, with potential for more depending on staffing
• 8-hour day shifts with every other weekend (EOW) requirement
• First to float; floating required to all units except surgery
• Cerner EMR used in this position

Requirements

• Active CA RN license required; pending licenses will not be accepted
• Minimum 2 years of RN experience required
• Current BLS certification required; pending certifications will not be accepted
• Recent experience as a Care Coordinator (RN) at a California Dignity Health hospital is required — candidates without this background will not be considered
• Two professional references required: one supervisor from within the last 12 months worked and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire
• Driver's license required for consideration

Additional Information

• Coordinates care for a high-volume, mixed-acuity patient population across multiple units
• One-day orientation provided with a Registered Nurse Care Coordinator (RNCC)
• Candidates who have been directly employed (full-time, part-time, or PRN) by any CommonSpirit, CHI, or Dignity Health facility within the past year are not eligible; those employed PRN through outside staffing firms may be considered
• RNs at this facility are required to pass a Relias Dysrhythmia exam (basic or advanced) before an offer can be confirmed
• A pass/fail sterile technique test covering Foley catheter insertion and central line dressing changes is administered on the first day of orientation
