American Traveler is seeking an experienced RN Hospice Case Manager for a community-based hospice role in the Lake Zurich, IL area, requiring a minimum of 2 years of hospice experience and Epic EMR proficiency.

Details

• Community hospice setting providing care to patients in their homes and at nursing facilities across the McHenry, Round Lake, Wauconda, and Antioch areas
• Patient population consists of hospice patients requiring acute symptom management, with a focus on end-of-life care and family support
• Daily patient visits of 5–6 patients with an overall caseload of 15–20 patients (ratio 1:5 daily visits, 1:20 caseload)
• Team utilizes Epic Remote Client and Rover for documentation; a laptop and cell phone are provided
• Monday–Friday, 8:00 AM–4:30 PM schedule with no weekend rotation, on-call, or holiday requirements
• Float coverage within the Red Team territory as needed for same-day assessments or sick call coverage
• 15-week contract

Requirements

• Active IL RN license required
• Current BLS certification required
• Valid driver's license and reliable personal vehicle required
• Minimum 2 years of hospice or end-of-life nursing experience, with most recent experience in that specialty
• Experience with Epic Remote Client required
• First-time travelers will not be considered for this position
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN dated within the last year required for consideration
• Candidate's date of birth, last 4 digits of SSN, and middle name or initial required for consideration

Additional Information

• Responsibilities include conducting daily home and facility visits, managing patient symptom needs, supporting families through end-of-life care, and adhering to hospice regulations
• Team support includes triage nurses, a team lead for escalations, and office staff
• One week of orientation with a preceptor is provided
• Navy scrubs required

• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
• RTO requests require Resource Team approval
