American Traveler is hiring an experienced RN for a Home Health Field RN position in Bastrop, TX, requiring HCHB and OASIS experience with a willingness to participate in on-call rotation.

Details

• Home health field setting serving patients across a wide central Texas service area
• Primarily geriatric patient population requiring in-home skilled nursing care
• Caseload of approximately 4–6 patients per day, averaging 24–30 points per week (equivalent to ~40 hours/week)
• Charting performed using Homecare Homebase (HCHB) EMR and OASIS — all charting must be completed while at the patient's home
• Monday–Friday, 8:00 AM–5:00 PM schedule
• Weekend availability required as needed
• On-call rotation required, including approximately 1 night of call per week (details discussed at interview)
• Travel up to 50 miles from home base required to cover assigned territory

Requirements

• Active TX RN license or Compact RN license — must be in hand at time of consideration (no pending licenses accepted)
• Current BLS certification required
• Minimum 1 year of home health experience required; minimum 2 years of overall clinical experience required
• HCHB and OASIS experience required; 1 year of experience with both preferred
• 2 professional supervisory references from within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration

Additional Information

• Responsibilities include administering skilled nursing care to patients in home environments, coordinating care with patients, families, certifying providers, and the interdisciplinary team, and developing and overseeing Plans of Care
• Must be a non-smoker due to patient sensitivity — the facility serves patients with COPD who are highly sensitive to cigarette smoke
• Scrubs in any color are acceptable
