American Traveler is seeking an experienced RN for a Float Home Health position in the South Shore/Plymouth County region of Massachusetts, requiring active MA licensure and at least 1 year of certified home care and case management experience.

Details

• Home health setting with patient visits conducted in patients' private residences across a broad South Shore and Plymouth County service area, including communities such as Brockton, Plymouth, Hingham, Scituate, Marshfield, Weymouth, and surrounding towns
• Patient population spans birth through geriatric, covering a wide range of clinical conditions managed in the home environment
• Shift hours are 8:00 AM – 4:30 PM
• Weekend rotation required once every 6 weeks
• On-call and holiday coverage required per departmental rotation
• EMR: Epic (charting experience required)
• 13-week contract

Requirements

• Active MA RN license required — no pending or conditional licenses accepted
• Minimum 1 year of certified home care experience required
• Minimum 1 year of case management experience required
• BLS certification required — AHA-issued only
• Reliable, independent means of transportation for home visits
• 2 professional references required for consideration
• Flu vaccine required; no religious or medical exemptions accepted

Additional Information

• Responsibilities include assessing patient conditions, developing and implementing individualized care plans, and providing ongoing education and support to patients and families
• Performs admissions processes and case management in compliance with MGB Healthcare at Home policies, including thorough electronic documentation and interdisciplinary communication
• Candidates residing within 60 miles of Massachusetts General Hospital in Boston are considered local and will be placed at a reduced pay rate
• Time and attendance tracked via UKG/Kronos
• No validated parking available at Boston-area locations; public parking is available at candidate's expense
