American Traveler is hiring a travel RN for a Hospice Admission Nurse role in a home health liaison setting, requiring 2 years of hospice experience and active Epic proficiency.

Details

• Community-based hospice role covering private homes, skilled nursing facilities (SNFs), and assisted living settings as part of a home hospice admission program
• Patient population consists of adults with end-of-life diagnoses including heart failure, cancer, and other terminal disease processes
• Average caseload of 2–3 patients per day depending on shift
• Monday–Friday schedule; no weekend rotation, on-call, or holiday requirements
• Shift options include 8-hour (0800–1630) or 12-hour (0800–2000) shifts; 12-hour schedules include rotating days off each week
• 40-hour standard work week
• Travel coverage primarily in Burlington and Walworth areas, with no more than 45 minutes between patient locations
• EMR: Epic (Hyperspace and Epic Remote Client) — experience required
• A laptop and phone will be provided for use in the field

Requirements

• Active WI RN license or compact RN license currently in hand
• Current BLS certification required
• Valid driver's license, personal vehicle, and active auto insurance required
• Minimum 2 years of hospice nursing experience required
• Epic EMR experience required
• Most recent experience must be within the last year
• At least 1 professional reference from a Supervisor, Manager, Director, or Charge RN collected within the last year
• Candidate DOB and last 4 digits of SSN required for consideration
• Candidate middle name or initial required for consideration

Additional Information

• Responsibilities include conducting hospice admissions, explaining hospice care and expectations to patients and families, collaborating with physicians for orders, and coordinating with liaisons and case managers
• Clinical duties may include performing interventions on the initial visit, symptom management for end-of-life care (pain, dyspnea), and educating providers on end-of-life medication management
• Support available from a supervisor/manager via phone and chat, the liaison team, and hospice medical directors
• Orientation includes new employee orientation (NEO) and 40 hours of unit-specific orientation with a staff member
• Business casual or professional scrubs are acceptable attire
• RTO requests must be approved through the manager
• Local candidates are accepted at the same rate — no radius restrictions apply
• Former employees of Advocate Health or any of its subsidiaries must have been separated for at least 12 months before being eligible
