American Traveler is hiring an experienced RN for a Home Health position in Oregon, requiring an active OR RN license and at least one year of acute care experience.

Details

• Day shift, 8:00 AM to 4:30 PM, Monday through Friday with a rotating weekend schedule (currently shared among 5 clinicians)
• Schedule is 5 days per week, 8 hours per shift, not to exceed 40 hours per week
• Weekend rotation includes a modified schedule: Thursday off the week prior, working Saturday and Sunday, with the following Friday off
• Patients are seen in their homes, which may include assisted living facilities or adult foster homes
• 13-week contract

Requirements

• Active RN license in the State of Oregon required
• Current BLS certification required
• Valid Oregon Driver's License and proof of current auto insurance required
• Must have personal vehicle available for use during work
• Minimum of one year of continuous RN experience in an acute care setting
• Graduate of an accredited school of professional nursing

Additional Information

• Assess patients' physical needs, develop individualized care plans, and manage ongoing care for patients recovering from illness or managing chronic conditions
• Collaborate and communicate with patients, families, caregivers, and interdisciplinary team members to coordinate the plan of care
• Provide patient and family education on disease and symptom management
• Initiate and revise the Plan of Care, make referrals to rehabilitative services, and coordinate care across disciplines
• Maintain timely and accurate documentation of assessments, care plans, treatments, and outcomes
• BSN and prior home health, community health, or critical care experience are preferred
• International candidates are not eligible for this position
• First-time travelers are not explicitly excluded, but at least one year of acute care experience is required
