American Traveler is seeking an experienced RN for a Home Hospice & Palliative Care position requiring a minimum of 2 years of home hospice RN experience and an active NJ license.

• Home health/hospice setting serving patients and families in a community-based environment
• Full-time schedule of 5 days per week, 8-hour day shifts (8a–4p)
• 26-week contract
• One weekend of on-call per month required
• Floating to different counties based on patient need
• EMR: Epic (experience required)

Requirements

• Active NJ RN license (NJ or Compact) required
• Minimum 2 years of RN experience in a home hospice environment
• ASN degree or RN diploma with active NJ licensure
• Current BLS certification
• Valid New Jersey driver's license
• Experience with EMR systems required
• Strong critical thinking and communication skills
• Two professional references required for consideration

Additional Information

• Responsibilities include conducting patient assessments to determine hospice appropriateness, developing and managing individualized plans of care for patients and families
• Performs case management duties including scheduling, care plan revision, and coordination with the interdisciplinary team (IDT), with IDT meetings held every two weeks
• Manages medication reconciliation, pharmacology knowledge, and safe medication practices
• Documents patient assessments, education, and visit notes via laptop with twice-daily data transfers
• Participates in performance improvement activities, utilization review meetings, and provides mentorship/orientation to new staff
• Candidates must reside more than 50 miles from the facility — local candidates are not eligible
• Travelers may float to a nearby sister facility as needed
