Description

American Traveler is seeking a licensed Physical Therapist (PT) for a Home Health assignment requiring at least 1 year of PT experience and a valid driver's license for patient home visits within an assigned territory.

Details

• Home health setting — clinician travels to patients' homes within an assigned geographic territory
• Caseload is managed within the assigned territory or surrounding areas to fill an 8-hour day
• Schedule is Monday–Friday, 8am–5pm, with no on-call requirements
• Homecare Homebase EMR experience is preferred
• Cerner is used for charting

Requirements

• Current SC PT license or compact license required; license required if applicable
• Current AHA BLS certification required
• Minimum 1 year of experience as a Physical Therapist preferred; home health experience strongly preferred
• Graduate of an accredited Physical Therapy program required
• Valid, current driver's license required — this position requires operating a vehicle
• Proficiency in English (speaking, reading, and writing) required
• Demonstrated knowledge of therapeutic tasks, rehabilitative activities, and orthotic/prosthetic/assistive devices
• Strong organizational and time management skills required
• 2 professional references required for consideration

Additional Information

• Primary responsibilities include evaluating, planning, and delivering patient care to address deficits in self-care, mobility, safety, range of motion, and strength
• Responsible for supervising and directing care provided by PTAs and Home Health Aides to ensure treatment goals are met
• Documents all care, coordination of services, orders, and goals in the patient's EMR accurately and in a timely manner
• Coordinates with other disciplines, agencies, families, and caregivers to meet patient care needs
• Candidates must live more than 50 miles from the facility to be eligible; local candidates are not accepted
• Travelers may float to a nearby sister facility
• Travelers may remain on assignment for up to one calendar year from their initial start date; a 30–60 day break is required before returning to a new assignment
