American Traveler is hiring an experienced RN Case Manager for an inpatient case management position requiring an active IL RN license and discharge planning expertise.

Details

• Inpatient hospital setting, Case Management unit
• Day shift schedule, 5x8-hour shifts (8:00 AM - 4:30 PM)
• Caseload includes inpatient admissions requiring psycho-social assessment, care coordination, and discharge planning
• Participates in Multidisciplinary Rounds as part of an interdisciplinary team

Requirements

• Active IL RN license required
• RN Case Manager experience required
• Experience with inpatient care coordination and discharge planning required
• 2 professional references from a supervisor within the past 2 years required for consideration
• Full 7-year work history with all gaps explained required for consideration

Additional Information

• Conducts initial patient and family psycho-social assessments and completes Care Coordination Assessments within the first 48 hours of admission
• Performs weekly documentation in patient medical charts, with more frequent updates as the patient's situation changes
• Communicates clinical status updates and treatment plan progress to patients, families, and other involved individuals
• Coordinates and finalizes discharge plans with the full rehabilitation team, reviewing instructions with patients and families prior to discharge
• All time-off and scheduling requests must be submitted at the time of initial consideration
