Description

American Traveler is seeking an experienced RN Case Manager for a 13-week Home Health assignment, requiring a minimum of 2-3 years of home health experience and an active MO or compact RN license in hand.

Details

• Home health setting serving geriatric patients with a focus on wound care and general home health cases
• Role functions as a Case Manager RN
• Caseload averages 4-6 patients per day, with 25-30 visits per week on a point-based system
• Monday through Friday schedule, 8:00 AM - 4:30 PM (5x8-hour shifts)
• No weekend requirements
• On-call may occasionally be required at the facility's discretion
• Charting system is Matrix; all charting must be completed at the time of the visit and cannot be taken home
• Territory and county coverage will be discussed during the interview process

Requirements

• Active MO or compact RN license, valid and in hand at time of consideration — no pending licenses accepted
• Current BLS certification required
• Minimum 2-3 years of home health nursing experience required
• At least 1 year of clinical management experience required
• Clean license with no hits or disciplinary actions
• 2 professional references from a supervisor within the past 2 years
• Full 7-year work history with all gaps explained

Additional Information

• Provides case management and skilled nursing care to home health patients, including wound care and geriatric populations
• Mileage is reimbursed at the IRS standard rate from first patient to last patient visit (home-to-first and last-to-home legs excluded)
• Blue scrubs and closed-toe shoes required
