American Traveler is seeking an experienced RN - Case Manager with a BSN and at least 3-5 years of acute care case management experience for a 13-week contract at a Trauma Level III teaching hospital in Concord, NC.

Details

• Hospital setting with 457 licensed beds, Trauma Level III designation, and recognition as a Chest Pain Center, Stroke Center of Excellence, and Bariatric Center of Excellence
• Case Management unit with a daily caseload of 14-16 patients, with the facility operating at approximately 99% capacity
• Patient population varies by assigned unit and may include Adult Medical/Surgical, Cardiac, Orthopedic, and Pediatric patients
• EMR: Epic (required); Careport for referral management (preferred)
• Rover Chat in Epic is used for communication; staff carry a phone while on the unit
• Schedule consists of five 8-hour day shifts per week with an 0800 start (0800–1630)
• No weekend rotation and no on-call requirements
• Floating to any unit is possible
• 13-week contract

Requirements

• Active RN license — NC or Compact license required
• BSN required
• BLS certification required
• Minimum 3-5 years of acute care case management experience required; most recent experience must be in this specialty area
• Epic EMR experience required
• Proficient in discharge planning, care coordination, and complex placement (SNF, IPR, LTAC, Hospice, Home Health, DME, Behavioral Health, DSS)
• Experience working with homeless and complex patient populations preferred
• Case Management certification preferred
• At least 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN
• Last 4 digits of SSN and full date of birth required for consideration

• Responsibilities include patient assessment, care coordination, discharge planning, and complex placement across a range of care settings
• Supported by a Director, Manager(s), CMO, a highly collaborative UM team (including a PA), peer Case Managers, and Social Workers
• Orientation length varies based on individual skillset and comfort level, ranging from 2–5 days of departmental orientation
• Department culture is described as teamwork-oriented with communicative management and knowledgeable, seasoned staff
• Required scrub color: Navy Blue
• Free parking is available on-site
• Holiday coverage is required unless time off is pre-approved
• RTO requires manager approval
• Candidates must live more than 50 miles from the facility to be eligible; local candidates are not accepted
• First-time travelers will not be considered; prior travel experience as an RN Case Manager is required
• Candidates who have worked a permanent position at an Advocate Health Southeast (AHSE) facility must have been separated for at least 1 year to be eligible
