American Traveler is seeking an experienced travel RN - Case Manager with at least 3-5 years of acute care case management experience and Epic proficiency for a 13-week contract at a Trauma Level III teaching hospital in Concord, NC.

Details

• Case Management unit within a 457-licensed-bed Trauma Level III teaching hospital that is also a Chest Pain Center, Stroke Center of Excellence, and Bariatric Center of Excellence
• Patient population includes adult medical/surgical, cardiac, orthopedic, and possibly pediatric patients depending on unit assigned
• Caseload of 14-16 patients per day, with the unit typically operating at 99% capacity
• EMR: Epic (required); Careport for referral management (preferred)
• Schedule consists of five 8-hour day shifts (8:00 AM – 4:30 PM) per week
• No weekend rotation and no on-call requirements
• Floating to any unit is possible
• Travelers are expected to work all holidays unless time off is approved

Requirements

• Active RN license — NC or Compact license required
• BSN required
• Current BLS certification required
• Case Management certification preferred
• Minimum 3-5 years of acute care case management experience required; most recent experience must be in this specialty
• Prior travel experience as an RN Case Manager required — first-time travelers will not be considered
• Proficiency with Epic EMR required
• Experience with SNF, IPR, LTAC, hospice, home health, DME, behavioral health, DSS (peds), and complex/difficult placement required
• Strong care coordination and discharge planning skills required
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum 1 professional reference dated within the past year from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Primary responsibilities include patient assessment, care coordination, discharge planning, and placement for a range of post-acute and community-based services
• Will routinely work with a homeless population and manage complex, difficult placements
• Will use Rover Chat within Epic and carry a facility phone
• Orientation ranges from 2-5 days depending on skillset and comfort level
• Department is supported by a Director, Manager(s), CMO, a highly supportive UM team (including a PA), peer Case Managers, and Social Workers
• Department culture is described as teamwork-oriented with communicative leadership and seasoned, knowledgeable staff
• Navy blue scrubs required

• RTO requires manager approval
• Candidates must reside more than 50 miles from the facility — local candidates are not accepted
• Candidates who have worked a permanent position at an Advocate Health Southeast (AHSE) facility must have been separated for at least 1 year before being eligible
