American Traveler is hiring an experienced RN for a Home Health position in Honolulu, HI, requiring at least 1 year of home health experience and OASIS documentation skills.

Details

• Home health setting serving an adult patient population in the Honolulu area
• Day shift schedule, 5x8-hour shifts (8:00 AM–4:30 PM)
• 25 units of productivity required per week
• No weekend or on-call requirements
• Occasional floating to cover Kauai, Maui, and/or the Big Island may be required, with all travel expenses covered
• MatrixCare experience is preferred for documentation

Requirements

• Active HI RN license required
• BSN preferred; ADN will be considered
• Current BLS certification (AHA)
• Minimum 1 year of home health experience required
• OASIS documentation experience of more than 3 months required
• Required clinical skills include: Adult Home Health, Admission Assessment, Case Management, Discharge Planning, Recertification, Resumption of Care, Routine Visits, Start of Care, Significant Change in Condition, Transfer, Blood Glucose/Glucometer Management, Colostomy/Ileostomy Care, G-Tube Site Care, Feeding Tube Management, NG Tube Care, Nutritional Assessment, Post AV Fistula/Shunt Care, Suprapubic Catheter Insertion/Management, Urinary Catheter Insertion/Management, Drain Management, Pressure Ulcer Prevention/Management and Staging, Wound Care, Wound V.A.C. Management, Medication Assessment/Management, Pain Management and Assessment, Pain Scale Utilization, Chemotherapy Side Effect Monitoring, Hospice Pain Management, Port Insertion/Care, Cast/Brace/Immobilizer Care, Post-Amputation Care, Post-Total Joint Replacement Care, Blood/Blood Product Administration, Peripheral IV/Central Line/Port Care, IV Start Skills, Phlebotomy, Clinical Assessment Documentation, Service Coordination, Infection Control, Vital Signs, and Oxygen Safety/Administration
• Preferred skills include: Psych Home Health, Tracheostomy Care, and MatrixCare documentation
• Closed-toed shoes required

• Responsibilities include providing comprehensive skilled nursing care to adult patients in their homes, including assessments, wound care, IV therapy, medication management, and care coordination
