American Traveler is seeking an experienced RN Case Manager/Navigator for a pre-assessment testing (PEEC) department role in Atlanta, GA, requiring an active GA or compact RN license and Epic EMR experience.

Details

• Regional office-based outpatient setting in Atlanta, GA
• Role is within a Pre-Assessment Testing (PEEC) department focused on navigating patients through their care journey
• Patient population includes individuals with chronic or complex conditions requiring coordinated, multidisciplinary care
• Monday through Friday schedule, 8-hour days (8:00 AM – 5:00 PM), 40 hours per week
• 8-week contract
• Epic EMR is used in this role

Requirements

• Active GA or compact RN license required — pending licenses are not accepted
• All certifications must be active at the time of consideration — pending certifications are not accepted
• Experienced RN Navigator background required
• Clinical skills including patient assessment, triage, symptom management, care planning, and data analysis are required
• Experience with Epic EMR
• Last 4 digits of SSN and date of birth required for consideration

Additional Information

• Serve as an RN Navigator guiding patients and families through diagnoses, treatment options, and available care resources
• Act as the primary liaison between patients, providers, families, and the broader care team to ensure coordinated, seamless care
• Perform triage, symptom management, data analysis, care planning, and evaluation for patients with chronic or complex conditions
• Local candidates are accepted — local and travel rates are the same
• Travelers who have previously worked at this health system should be aware that assignments are subject to a two-year maximum tenure policy, after which a 90-day break is required before returning
