American Traveler is seeking an experienced RN - Case Manager in California with at least 2 years of case management experience and utilization management background for a 4x10 day shift role.

Details

• Acute care hospital setting, Case Management unit
• Collaborates with physicians, utilization managers, medical social workers, bedside RNs, interdisciplinary teams, third-party payers, and community resources to coordinate patient care
• May be assigned to the Emergency Department to address complex clinical and social situations to avoid unnecessary admissions
• Schedule: 4x10-hour day shifts (0800–1830) with rotating weekends
• No on-call requirement

Requirements

• Active CA RN license required
• Minimum 2 years of case management experience
• Utilization management experience working with attending physicians to support level of care protocols
• 2 supervisor references from within the past two years required for consideration
• Copies of all certifications required for consideration

Additional Information

• Responsible for care coordination and care transitions planning throughout the acute care patient experience, ensuring timely progression and transition to the appropriate level of care
• Facilitates communication and care coordination across the continuum to achieve optimal health outcomes, appropriate utilization of resources, and patient self-determination
• Candidates must reside more than 50 miles from the facility to qualify for travel rates; local candidates within 50 miles should contact their account manager regarding local rate options
• Former employees or travelers must have been away for at least 1 year before returning as a traveler
• Only 1 holiday RTO request will be approved for this assignment
