American Traveler is seeking an experienced RN Case Manager for a home health position requiring active CA RN licensure, OASIS documentation experience, and a background in home health case management.

Details

• Home health setting with a designated patient caseload in the field
• Day shift schedule, 5x8-hour shifts (8:00 AM - 5:00 PM)
• 13-week contract
• Case management role focused on in-home nursing care coordination — hospital case management experience does not apply to this role

Requirements

• Active CA RN license required
• Current BLS certification from the American Heart Association (AHA)
• Minimum 2 years of RN experience required
• Recent home health case management experience required — hospice experience does not satisfy this requirement
• OASIS documentation experience required
• Epic EMR experience required

Additional Information

• Responsible for assessment, planning, coordination, and evaluation of in-home nursing care for an assigned patient caseload
• Develops and manages individualized, interdisciplinary care plans incorporating clinical, psychosocial, spiritual, economic, and lifestyle factors
• Coordinates with physicians, interdisciplinary team members, and families to ensure continuity and quality of care
• Manages visit utilization and expense control across the assigned caseload
• Accurately documents all patient care and treatment information in the EHR
• Candidates must reside within 50 miles of the home office to be eligible for the travel pay rate
• Previous employees must have been separated for at least 1 year before returning as a traveler
• Only 1 holiday RTO request will be approved
• 2 supervisory references from within the past two years are required for consideration
