American Traveler is seeking a licensed RN or LMSW for a Care Manager II position in an acute hospital Case Management department, requiring case management experience including utilization review, MCG/InterQual criteria, and Epic proficiency.

Details

• Hospital-based Case Management unit in an acute care setting
• 5-day, 8-hour day shifts (8:00 AM–5:00 PM), with possible early start at 7:30 AM; must remain until all cases are completed for the day
• 13-week contract
• Uses Epic EMR (experience required)

Requirements

• Active RN or LMSW license in Texas (or compact RN license) required
• LBSW accepted for candidates with 5+ years of demonstrated success in a CHRISTUS Care Manager I role
• Minimum 2 years of clinical experience required; 1 year in an acute care setting preferred
• Experience in case management, utilization review, MCG criteria, and InterQual criteria required
• Acute hospital case management experience highly preferred
• BSN or MSW required; demonstrated success in a CHRISTUS Care Manager I role for 5+ years may be considered in lieu of degree
• Certification in Case Management (CCM or equivalent) preferred
• BLS preferred
• Proficiency in Epic required
• Strong verbal and written communication skills with the ability to interact with diverse populations
• Ability to move throughout the hospital for the majority of the workday
• SSN required for consideration

Additional Information

• Responsible for care coordination and discharge planning from admission through discharge, including assessment, planning, implementation, and evaluation of individual patient needs
• Collaborates with patients, families, physicians, and multidisciplinary team members to manage patient progression, length of stay, and safe transitions across the continuum of care
• Identifies and resolves barriers to care, escalates quality and risk issues to Physician Advisors or the CM Director as appropriate
• Coordinates post-acute placement and community resource referrals including skilled nursing, home health, hospice, rehab, psychiatric, and DME services
• Actively participates in Multidisciplinary/Patient Care Progression Rounds and clinical performance improvement activities
• Documents in the medical record per regulatory and departmental guidelines
• May serve as preceptor or orienter for new associates and may assist with special projects
• Temp-to-perm opportunity available
