Description

American Traveler is seeking an experienced RN for a Hospice Case Management position requiring a MA RN license and the ability to conduct 4-5 outpatient visits per day across facilities and patient homes.

Details

• Outpatient hospice case management setting, with visits conducted in patient homes and care facilities
• Caseload of approximately 4-5 patient visits per day
• Daily travel of approximately 30-50 miles to reach patients in the surrounding area
• Schedule is Monday–Friday, 8:00 AM–4:30 PM (8-hour day shifts)
• 13-week contract

Requirements

• Active MA RN license required and must be in hand at time of consideration
• Experienced hospice RN background required
• Flu vaccination required prior to start (medical exemptions allowed)
• COVID vaccine and booster required (medical exemptions allowed)
• Must pass Telemetry/ECG and Medication competency tests

Additional Information

• Responsible for conducting routine hospice visits in outpatient facilities and private residences
• Candidates must reside more than 50 miles from the facility — local travelers are not eligible for this position
• Candidates who have worked for Beth Israel Lahey Health (travel, permanent, or per diem) within the last 6 months are not eligible — a 6-month separation from any Beth Israel Lahey Health facility is required
• All requested time off (RTO) must be disclosed at the time of consideration
