American Traveler is hiring an RN Care Manager for a hospital-based role requiring a compact or ME RN license and significant experience in care management and discharge planning.

Details

• Work in a hospital setting with responsibilities spanning all inpatient units
• 40 hours per week, with variable shift options: either 8:00 am – 4:30 pm or four 10-hour shifts from 7:30 am – 5:00 pm
• No on-call required
• Weekend and holiday rotation required
• Coordinate utilization management activities throughout the hospital

Requirements

• Active ME or compact RN license required
• Associate or Bachelor degree in Nursing required
• Minimum of 3 years clinical experience in med/surg nursing, home care, hospital case management, and/or utilization review required
• Current BLS certification required
• CCM or ACM certification preferred
• Utilization review and discharge planning experience required
• Strong written, verbal, and electronic communication skills necessary

Additional Information

• Responsible for assessing patient needs and transitioning patients to appropriate care settings
• Collaborate with and lead an interdisciplinary team in care planning and daily rounds
• Arrange for patient resources including equipment, transportation, and insurance authorizations
• Participate in managing length of stay and understanding of hospital quality measures
• Work with community organizations to support patient care transitions
