American Traveler is seeking a Case Manager RN with at least one year of acute care, discharge planning, and care coordination experience, who holds a WA or compact RN license.

Details

• Position is based within a Level II Trauma Center across two hospital campuses
• Unit assignment is Case Management covering all areas including Med Surg, ED, and those with long length of stay
• Day shift schedule with 5x8 hour shifts from 7:30 to 16:00
• Weekend coverage required every other weekend
• Floating required between all hospital units and campuses
• Uses Epic EMR system

Requirements

• Active WA or Compact RN license required
• BLS certification required
• Minimum of 1 year of acute care RN experience
• Experience in discharge planning and care coordination required
• Diverse case management experience in Med Surg, long length of stay, and ED strongly preferred
• Minimum of 2 supervisory references covering at least 1 year within the last 3 years from a comparable specialty or setting
• Epic EMR experience required
• First time travelers accepted
• Local candidates not accepted
• Former permanent staff must wait 3 months before returning and reside more than 50 miles from facility

Additional Information

• Responsible for comprehensive case management, including patient assessment, discharge planning, and coordination of care across the continuum
• Expected to float between all facilities within the state as needed
• Minimal to no RTO preferred; all requests for time off must be included at the time of application
• Required to adhere to hospital dress code; any scrub color except reddish orange is allowed
• Facility provides paid parking
