Description

American Traveler is seeking an experienced Hospice RN with an active UT or compact RN license for a day shift home health role in a community-based hospice setting.

Details

• Work in a hospice setting providing care primarily in patients' homes
• Responsibilities include comprehensive assessment and individualized care planning for terminally ill patients
• Day shift position with on-call rotation required 1-3 shifts per week, including coverage of 3 weekends over the 13-week contract
• Expectation to respond within 45 minutes to patient needs when on call
• Experience with Hospicesoft or Careficient EMR is preferred

Requirements

• Active RN license in Utah or a compact state
• Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care
• Current BLS certification required
• Valid driver’s license and reliable transportation required for home visits
• Strong clinical assessment, communication, and organizational skills

Additional Information

• Responsibilities include managing pain and symptoms, educating and supporting patients and families, and collaborating with the interdisciplinary hospice team
• Must complete and maintain accurate patient documentation according to policy standards
• Holiday coverage is required as assigned
• Mileage reimbursement is provided for travel to patient homes
• Blue scrubs are preferred for this role
• The position requires successful completion of drug testing and background check
