Description

American Traveler is seeking an experienced Hospice RN with at least one year of skilled hospice experience and HOPE documentation proficiency for home health assignments in Medford, OR and surrounding areas.

Details

• Work within a home health hospice setting visiting patients in Medford, OR and surrounding communities
• Patient assignments are 1:1, with up to six home visits per day and a productivity expectation of 30 points
• Day shift schedule of five 8-hour shifts per week, from 8:00 AM to 4:30 PM
• No weekend or on-call duties required
• No floating required between units, but floating may occur between facilities within the state
• EMRs used include Epic and HOPE; HOPE documentation experience is required
• Any color scrubs may be worn on assignment
• Mileage reimbursement is provided at the IRS standard rate

Requirements

• Active OR RN license or eligibility for licensure in Oregon
• Current BCLS certification
• Minimum one year of skilled hospice RN experience
• Proficiency with HOPE documentation system required
• Experience using Epic EMR preferred
• Primary COVID-19 vaccination series and at least one booster required
• Reliable personal transportation is required
• Former permanent staff must be separated for at least three months and reside more than 50 miles from the facility to be eligible
• Locals are accepted and defined as candidates residing within a 50–75 mile radius

Additional Information

• Provide comprehensive hospice care and case management to patients in their homes
• Client groups patient assignments to minimize drive time, though occasional travel up to 50+ miles may be required
• All travelers may be asked to float between facilities within the state if needed
• Two supervisory references covering at least one year within the past three years in a similar setting are required
• Modules averaging 16 hours must be completed pre-start and annually
• Minimal RTO is permitted and must be discussed during consideration; additional RTO may affect assignment eligibility
• Free parking is available for staff
• First-time travelers are not accepted for this role
