American Traveler is seeking an experienced RN Case Manager with at least 5 years of complex case management and utilization management experience for a critical need position in Atlanta, GA.

Details

• Position based in a regional office setting
• Full-time schedule, 8:30 AM to 5:00 PM, 8-hour shifts
• Includes weekend and holiday scheduling
• Assignment length is 13 weeks
• Role focuses on case management of patients with chronic conditions such as diabetes, cardiac issues, and behavioral health challenges
• Uses EPIC EMR system

Requirements

• Active GA or compact RN license required at time of consideration
• Minimum of 5 years’ experience in case management and utilization management
• Experience with complex case management and certification in this specialty preferred
• Experience managing patients with chronic conditions including diabetes, cardiovascular disease, and behavioral health
• Familiarity with EPIC EMR system preferred
• Must provide last 4 digits of SSN and date of birth for consideration

Additional Information

• Responsible for managing and coordinating care plans for patients with complex and chronic conditions
• Weekend and holiday work is required as part of the regular schedule
• Local and travel candidates considered at the same rate
• Assignment capped under health system policy at two years, after which a 90-day break is required before subsequent assignments
