Description

American Traveler is seeking an experienced RN Case Manager with a CA RN license and BLS certification for a day shift position in an acute care hospital setting.

Details

• Hospital-based case management role
• Day shift schedule, 5x8 hour shifts from 8:00 to 16:30
• Required to work every other weekend
• May be assigned to Emergency Department as needed
• 50 mile radius rule applies for eligibility

Requirements

• Current and active CA RN license
• Current Basic Life Support (BLS) certification
• Minimum 5 years of RN case management experience required
• Must not have been employed at the facility within the past year to be eligible as a traveler

Additional Information

• Leads care coordination and transition planning throughout the acute care patient experience
• Collaborates with interdisciplinary teams including physicians, social workers, and utilization management
• Responsible for patient assessment, utilization management, and documentation in EHR systems
• Coordinates discharge planning, identifies risk for readmission, and arranges appropriate post-acute services
• Focuses on complex clinical and social situations to prevent unnecessary admissions
• Must be available to complete lab work within 3-5 days of contract acceptance
• Only one holiday request for time off will be approved during assignment
