American Traveler is seeking an experienced RN Case Manager for a primary care clinic in Alaska, requiring an active AK RN license and recent case management experience.

Details

• Primary care clinic setting with a hybrid role of case management and clinic nursing
• Patient population includes acute and chronic cases with health maintenance responsibilities
• Work schedule is Monday through Friday, day shift
• Position involves both patient case management and direct clinic nursing duties

Requirements

• Active, unrestricted Alaska RN license required at time of consideration
• Graduation from an accredited nursing program required; BSN preferred
• Current BLS certification required
• Minimum 1 year of case management experience within the last 2 years
• Ability to move quickly, stand for long periods, and perform physical nursing tasks
• National Case Management certification preferred
• Proof of flu vaccine required prior to onboarding
• Fingerprint cards required for onboarding

Additional Information

• Role requires active management of assigned patients and serving as a clinic nurse as needed
• Collaboration with providers and interdisciplinary team members to set and achieve individual patient goals
• Coordinates care conferences and manages care for high-risk or complex patients
• Facilitates and tracks patient referrals within and outside the organization
• Maintains annual competencies, safety, and infection control standards
• Must live at least 50 miles from the facility; locals not accepted
• No housing or travel provided; all-inclusive rate applies
• Participates in team meetings and quality improvement initiatives
• Annual competency and infection control compliance required
