Description

American Traveler is seeking a highly experienced Integrated Case Manager RN with at least 5 years of recent acute inpatient case management and discharge planning experience for a hospital-based role requiring a current MI RN license.

Details

• Position is based in an inpatient case management unit at a hospital
• Typical schedule is days, 8:00 am to 4:30 pm, with rotating weekends
• Patient caseload typically ranges from 21 to 25, with direct contact determined by individual risk factors
• Role requires floating to a sister hospital as needed
• Epic EMR is used in this setting, and prior experience is required

Requirements

• Active and unrestricted MI RN license or MI LMSW license required
• Minimum of 5 years of recent acute inpatient case management experience, including acute discharge planning
• BLS certification through the American Heart Association required
• Degree in Nursing or MSW required
• Strong computer and Epic EMR skills required
• Excellent verbal communication, written documentation, and customer service skills
• Knowledge of Medicare and Medicaid regulations, clinical practice guidelines, and case management principles
• COVID-19 vaccination required

Additional Information

• Responsible for comprehensive assessment, care planning, care coordination, and advocacy for patients with complex healthcare needs
• Engages in discharge planning and coordinates seamless transitions of care for patients
• Facilitates patient and family engagement, provides education, and advocates for service delivery within benefit structures
• Utilizes motivational interviewing and self-management strategies to address barriers to care
• Provides counseling on advanced care planning and intervenes in cases such as abuse or neglect
• Online modules must be completed onsite prior to the start date
• No block scheduling available
• Holiday coverage may be required based on rotation
• RTO must be requested prior to assignment; requests after start are subject to management approval
• Requires successful completion of background check and drug screen prior to start
