American Traveler seeks an experienced RN for a day-shift hospice home care position requiring HCHB and travel experience.

Details

Home care hospice setting with field visits throughout the day
Day shift schedule, Monday through Friday, 8:00 AM to 5:00 PM
Responsible for managing a caseload and providing case management services
Mileage reimbursement provided at the federal rate

Requirements

Active NC or compact RN license required
At least 1 year of prior case management experience required
Prior experience with HCHB (Homecare Homebase) and travel nursing required
Must have a valid driver's license and personal vehicle

Additional Information

Provides hospice care to patients in their homes across assigned territory
Daily field travel required to multiple patient locations
Two professional references must be provided for consideration
This position does not involve inpatient care
