Description

American Traveler is seeking an experienced RN for an adult case management position requiring two years of inpatient case management experience and Epic proficiency.

Details

• Work in an adult medicine/surgery inpatient case management unit
• Day shift schedule: Monday through Friday, 8:30 AM to 5:00 PM
• No weekends, holidays, or on-call requirements
• Patient caseload includes coordination from triage through discharge to various post-acute settings
• EMR system used is Epic, with additional use of referral platforms such as NaviHealth or AllScripts

Requirements

• Active MD or compact RN license
• Minimum two years RN experience
• Current inpatient case management experience in a hospital with at least 500 beds
• Basic Life Support (BLS) certification is required
• Experience with Epic EMR and post-acute facility referral platforms (NaviHealth or AllScripts) required
• Candidates must have a current/active CPR card

Additional Information

• Coordinate care, transition planning, and safe transfers to home or post-acute settings for a select patient caseload
• Communicate proactively with payors and the multi-disciplinary team to optimize care plans and resource approval
• Maintain clinical system and fiscal data related to specific patient populations
• Enter and maintain patient information in all departmental systems following guidelines
