Description

American Traveler is seeking an experienced RN for a Case Manager position requiring BLS certification, MO or compact license, and acute hospital care coordination experience.

Details

• Work within the case management unit of a hospital setting
• Responsibilities include adults needing home health setup and discharge assistance
• Shift is Monday through Friday, 8:00 am to 5:00 pm, with some Saturdays as needed
• Floating within scope may be required to meet facility needs
• Uses Epic EMR, with required experience
• No on-call responsibilities

Requirements

• Active MO or compact RN license required
• Basic Life Support (BLS) certification required
• 1-2 years of acute hospital experience in care coordination, discharge planning, and needs assessment
• No first-time travelers will be considered
• Must reside outside a 75-mile radius from the facility
• Strong typing, Microsoft Office, and Epic documentation skills required
• Excellent communication and organizational skills

Additional Information

• Main duties involve care coordination, discharge planning, and ensuring clear documentation
• Must be able to work in a fast-paced environment and prioritize tasks effectively
• Travelers are required to work holidays as necessary
• No more than 7 days of requested time off permitted per 13-week contract, and no time off allowed during the first week of orientation
• Permanent employees must wait one year before returning as travelers
• Must take a three-month break after 18 months of continuous assignment as a traveler
• Pre-start training modules to be completed during orientation
