Description

American Traveler is seeking an experienced RN with 2-3 years of home hospice experience for a Hospice and Palliative Care position requiring an active NJ or compact RN license and BLS certification.

Details

• Work in the Homecare Hospice and Palliative Care setting
• General inpatient (GIP) hospice unit
• Full-time schedule with 11am – 7pm shifts, every other weekend required
• Patients include those needing hospice and palliative care services
• Floating to a nearby sister hospital may be required
• Uses Epic for electronic medical records

Requirements

• Active NJ or compact RN license required
• Minimum of 2-3 years RN experience in a home hospice environment required
• Current BLS certification required
• Valid New Jersey driver’s license required
• Previous ICU experience preferred
• Experience with EMR charting, preferably Epic
• Excellent critical thinking and communication skills necessary
• Candidates must live at least 50 miles from the facility; local candidates are not eligible

Additional Information

• Perform patient assessments and develop individualized care plans for hospice patients and their families
• Manage case loads, ensure appropriate utilization of hospice services, and maintain visit productivity standards
• Complete and document assessments, admissions, discharges, and revisit documentation within specified timeframes
• Provide patient and family education using age-appropriate methods and document educational tools used
• Attend multidisciplinary team meetings and participate in quality improvement activities
• Responsible for safe medication management, routine medication reconciliation, and maintaining current medication orders
• Participate in orientation and mentorship of new employees and cross-train as needed
• Follow infection control and safety procedures and report any infections as necessary
• Must attend monthly staff and periodic PI meetings
• Assignment duration limited to one calendar year; a 30-60 day break is required before returning
