American Traveler is seeking an experienced RN Care Coordinator for a day shift position requiring a CA RN license, BSN, and cardiac/heart transplant experience.

Details

• Ambulatory care setting with a focus on heart failure and heart transplant patients
• Day shift schedule with 8-hour shifts, 5 days per week from 8:00 to 16:30
• Responsibilities include managing immunosuppression, prophylactic antibiotics, infection, organ rejection, and routine monitoring
• Floating required to other facilities within a 75-mile radius as needed

Requirements

• Active CA RN license required
• Bachelor of Science in Nursing (BSN) required
• Current AHA Basic Life Support (BLS) certification required
• Minimum 1 year of recent ambulatory care experience required
• Experience in cardiac nursing including management of heart failure and/or heart transplant recipients required
• Must have prior travel assignment experience within the last 12 months
• COVID vaccination and booster required, with documentation
• Ability to float to other departments or facilities as needed

Additional Information

• Main responsibilities include comprehensive care coordination for heart transplant patients
• Will monitor patients for immunosuppression management, infection, organ rejection, and all routine post-transplant needs
• Experience with Phoenix EMR and management of other organ transplant recipients preferred
• Working knowledge of immunosuppressive therapies preferred
• Must complete mandatory modules pre-assignment and annually
• Prior experience in a teaching hospital preferred
• First time travelers will not be considered for this position
