American Traveler is seeking an experienced RN Case Manager for a hospital-based care coordination and discharge planning position requiring a CA RN license and acute care experience.

Details

• Position is based in an acute care hospital setting within the Case Management department
• Day shift schedule, 5x8-hour shifts from 8:00 to 16:30
• Patient ratios are typically 1:20
• No floating required for this specific assignment, but all travelers may be expected to float between facilities statewide if needed
• Weekend rotation is required as needed by the department
• Epic EMR experience required or must be willing to learn

Requirements

• Active CA RN license required (pending license accepted if issued before start)
• Minimum 2 years of acute care hospital Case Manager experience with care coordination and discharge planning
• Experience in a hospital setting is required
• Current BLS certification required
• Must provide two supervisory references covering one year of employment within the last three years in a comparable specialty or setting
• SSN and DOB required for consideration

Additional Information

• Responsible for comprehensive care coordination and discharge planning for acute care patients
• Must adhere to department and facility policies on time off, with a maximum of 7 days RTO allowed
• First-time travelers are welcome and will be screened by clinical staff
• Scrub or business attire is acceptable per department policy
• Former permanent staff must wait 3 months before returning as a traveler and must live at least 50 miles from the facility
• All requested time off must be disclosed at submission; changes may be denied post offer
