American Traveler is hiring an experienced RN Case Manager for adult medicine/surgery case management with current inpatient experience and day shifts.

Details

• Work in an adult medicine/surgery case management unit in a hospital setting
• Day shift, Monday to Friday, 8:30 AM to 5:00 PM
• No weekend, holiday, or on-call requirements
• Caseload includes coordinating care and safe transition planning for adult inpatients
• Utilizes EPIC EMR and systems for post-acute referrals such as NaviHealth or AllScripts

Requirements

• Active RN license (MD or compact)
• Minimum two years of RN experience required
• Current BLS certification required
• Current inpatient case management experience required
• Hospital experience in facilities with 500+ bed count required
• Experience with EPIC EMR and post-acute referral platforms preferred

Additional Information

• Coordinate care from triage through discharge including communication with payors to secure resources
• Collaborate with multidisciplinary team and patient/family for effective transition to post-acute settings such as skilled nursing, inpatient rehab, or local shelters
• Maintain and enter patient information in all relevant departmental systems according to guidelines
• No medication administration responsibilities or med testing required for this role
• A current CPR card is required for bedside case managers
• Local candidates are accepted at the same rate except for Howard County General, which has a 60-mile radius rule with no locals allowed
